Outpatient Sickness Rider |
|
$50 benefit paid up to four
times per calendar year for treatment of a sickness in a physician’s
office or outpatient facility. Pays twice the benefit amount for
emergency room treatment of sickness. |

Annual Wellness Benefit Rider |
|
Pays per calendar year up to the
following amounts for: annual flu shot and pneumonia immunization =
$20 per shot (max. one each per year); routine dental visits = $25
per exam (max. two per year);
hearing aid = up to $100 per purchase (one hearing aid each two
years) annual hearing exam = $40 per exam (one exam per year);
prescription lenses and frames = $50 every two years;
annual eye exam = $40 per exam (one exam per year);
annual physical exam = $100 per exam (one exam per year). |
