| Schedule of Benefits and Losses |
| |
| Benefits are paid for a covered accidental bodily injury as a percentage of the Principal Sum you select, according to the following: |
| |
| Description of Loss | % of Principal Sum |
| | Loss of Life | | 100% | | Loss of Any Two - Hand, Foot or Eye | | 100% | | Loss of Speech and Hearing in Both Ears | | 100% | | Quadriplegia | | 100% | | Paraplegia | | 50% | | Loss of One Hand, Foot or Eye | | 50% | | Loss of Speech or Hearing in Both Ears | | 50% | | Hemiplegia | | 50% | | Loss of Thumb and Index Finger of the Same Hand | | 25% | | |
|
| |
| Quadriplegia means total paralysis of both upper and lower limbs; paraplegia means total paralysis of both lower limbs; and hemiplegia means total paralysis of upper and lower limbs on one side of the body. |
| |
Loss means: - for a hand or foot, complete severance through or above the wrist or ankle joint;
- for an eye, the total, permanent and irrecoverable loss of sight in the eye;
- for a thumb and index finger, complete severance through or above the metacarpophalangeal joints;
- for speech, total, permanent and irrecoverable loss of audible communication;
- for hearing, total and permanent loss of hearing in both ears which cannot be corrected by any means.
| | | | You must be covered under the plan at the time of the accident. Bodily injuries suffered must be as a direct result of the accident and from no other cause. Death or injury must occur within 1 year of the accident. If more than one loss results from any one accident, the loss with the largest benefit amount is payable. Each covered person is limited to one total Principal Sum amount for all covered accidents incurred while covered under this plan. |

| Education Expense Benefit - When you die from a covered accident, this benefit provides 2% of your Principal Sum up to $7,500 per year, for 4 consecutive years towards the cost of an accredited school beyond the 12th grade for each of your covered dependent children who are eligible. Eligible children are those under age 25 who are attending an accredited school beyond the 12th grade at the time of the accident, or those who enroll within one year from the accident. This benefit is paid in addition to all other benefits of the plan. |
| |
| Seatbelt Use Benefit - An additional 10% of a covered person's Principal Sum is payable for a covered accidental death resulting from a private passenger car accident. | Other |
| Other Advantages |
| |
| Portable - once you have been insured for 2 years, this coverage will not terminate just because you are no longer eligible due to change in job. |
| |
| Affordable Coverage - accident only coverage provides an affordable opportunity to reassess your insurance needs if you've recently married, divorced, started a family, sent children to school or started paying on a mortgage or large loan. |
| |
| Automatic Payroll Deduction - your coverage will be automatically deducted from your paycheck on a monthly or tenthly basis, as selected by you. |
| |
| Family Coverage - You can select a benefit plan that includes your eligible family members. If spouse coverage is later terminated, that amount is added to your coverage. If coverage for all of your children is later terminated, twice their amount is added to your Principal Sum. The $300,000 maximum on the Principal Sum still applies to these circumstances. |
