Q: Who is eligible for coverage?

A: Any employee in good standing of the of California Community Colleges will be eligible to apply for coverage provided they are under age 60, not in full-time service of the armed forces, are a resident in the US, and are actively engaged half-time or more in the duties of their occupation. ("Half-time" means that you are regularly working at least one-half of the number of hours necessary to fulfill your contractual requirements with an eligible employer as defined by Faculty Association of California Community Colleges' by-laws.)

Q: What is not covered?

A: Benefits will not be paid for any total disability that results from: intentionally self-inflicted injuries or suicide while sane or insane; war or acts of war, whether declared or not; injury or sickness while on full-time active military duty for more than 30 days; travel or flight in, or getting in or out of an aircraft that is being used for test experiment or that you are flying, learning to fly, are a crew member of, or a military aircraft (other than MAC); normal pregnancy or childbirth; pre-existing conditions (refer to the Pre-Existing Conditions section in the plan details).

Q: Will I have to pay premiums while I am totally disabled?

A: Premiums will be waived, beginning with the first premium due date after benefits have been paid for 6 straight months, while you remain totally disabled.

Q: When will coverage begin and end?

A: Coverage will begin on the first of the month following the approval of your application and receipt of your first premium by the Administrator. Since medical questions are asked, it may be necessary to contact your physician(s) to clarify any medical condition that you may currently have. You will be advised when and if coverage is approved. If on that date you are not actively engaged half-time or more in the duties of your occupation or profession, your coverage will be deferred until you resume your normal duties. While the group policy remains in force your coverage will continue provided you remain in an eligible class; you pay the premium when due; you are actively at work at least half-time in your occupation or profession; and you do not enter military service for more than 30 days active duty. Coverage will end when you retire, reach the maximum age (see rate section for specific termination age), are no longer working half-time or more at the duties of your job or profession; or when you have been paid benefits for the maximum period for one disability.

Q: Am I eligible for coverage?

A: Yes, if you are a member in good standing of the Faculty Association of California Community Colleges who is under age 60, living in the US, not in full-time service of the armed forces and actively engaged at least half-time or more in the duties of your profession (“Half-time” means that you are regularly working at least one half of the number of hours necessary to fulfill your contractual requirements with an eligible employer as defined by Faculty Association of California Community Colleges by-laws.). Your spouse under age 60 and unmarried, dependent children age 14 days (or out of the hospital, whichever is later) through 24 years who principally reside with and rely on you for support and maintenance are also eligible if they reside in the US, are not in full-time military service and are performing the normal duties of persons of like age and sex and are not disabled.

Q: Can I convert coverage to another policy?

A: If, before you reach age 70 and you have been covered under the same policy for at least 5 years (if group policy is terminated or amended to terminate coverage for your class), your coverage ceases for any reason except non-payment of premium, you can convert to an individual policy. No medical certification is needed.

To continue coverage, you must apply for the conversion policy and pay the first premium in effect for your age and occupation within 31 days after your group coverage ends. Family members may convert their coverage as long as they have not reached the maximum age limitation. Converted policies are subject to certain benefits and limits as outlined in the policy, should you become insured under the plan.

Q: Does this plan cover death for any reason?

A: The plan provides benefits for death at any time, in any place and due to any cause, except while on active duty in the armed forces or suicide (while sane or insane) in the first two years of coverage.

Q: When will coverage begin and end?

A: Coverage will become effective on the first of the month coinciding with or immediately following the date your application is approved and premium has been received by the Administrator. You must be actively engaged half-time or more in the duties of your profession on the effective date or coverage will be deferred until you resume at least the half-time duties of your profession. Your dependents must be performing the normal duties of persons of like age and sex and not totally disabled on the effective date or coverage will be deferred until they are performing normal duties and are not totally disabled.

Coverage will continue provided you pay your premium when due, the group policy stays in force and you remain an eligible member. Coverage will terminate on the premium due date following your 70th birthday or the date that your eligibility class or plan of benefits is terminated under the group policy. Dependent coverage will continue as long as you are covered under the plan, they remain an eligible dependent and premium is paid on time provided that the group policy is in force and coverage has not been terminated for the dependent’s class of eligibility.

Your legal spouse under age 60, and dependent children under age 25 who principally reside with and rely on you for support are also eligible. All dependents must reside in the US.

Q: Are there any exclusions or limitations?

A: No benefits will be paid for losses which are caused by or result from intentionally self-inflicted injury or suicide (while sane or insane); voluntary self administration of any drug or chemical substance not prescribed by and taken according to the directions of a doctor (accidental ingestion of a poisonous substance is not excluded); a loss incurred driving while intoxicated or driving under the influence of a controlled substance unless administered on the advice of a doctor; commission or attempt to commit a felony; participation in a riot or insurrection; declared or undeclared war or act of war; an act of terrorism; while on active duty of the armed forces; travel or flight in any aircraft or device which can fly above the earth's surface if it is being used for test or experimental purposes, by or for any military authority, for travel or is designed for travel beyond the earth's atmosphere, by or for your association, its subsidiaries and affiliates or if the insured is serving as a pilot or crew member; hang-gliding, parasailing or parachuting; sickness, disease, bodily or mental infirmity or medical or surgical treatment thereof, or bacterial or viral infections, regardless of how contracted (this does not include bacterial infection that is a natural and foreseeable result of an accidental external bodily injury or accidental food poisoning); or participation in any organized race or speed contest.

Q: When will coverage begin and end?

A: Coverage will become effective on the first of the month coinciding with or following the date after your enrollment form is accepted and premium has been received by the Administrator. Coverage for your dependents will become effective under the same conditions. Coverage will continue as long as you remain eligible under the plan (not applicable after covered for more than 2 years), the group policy stays in force, and premium is paid on time. Coverage will terminate on the date your eligibility class or your plan of benefits is terminated or the premium due date following your 70th birthday. Dependent coverage will continue as long as they remain an eligible dependent and, while you are covered, under the same conditions applicable to you.
